The bay holds roughly 1,600 limestone islands and islets, most of them uninhabited. They are the tops of a karst landscape like Ninh Bình's, formed on land over millions of years and flooded when the sea rose after the last ice age. The pillars and domes are called fengcong and fenglin karst by geologists: clusters of peaks on a shared base, and isolated towers standing on the sea floor.

Wave action has cut deep notches at the waterline of many islands, giving them their undercut, mushroom look. Inside, caves formed when the land was dry, which is why Sửng Sốt and Thiên Cung have stalactites far above today's sea level.

The bay is shallow, mostly under 10 metres, and sheltered, which kept it calm enough for small boats but also makes it sensitive to pollution from moorings, coal ports and urban growth around Hạ Long city.

Archaeologists have found shell middens and stone tools from the Soi Nhụ and Hạ Long cultures on the islands, showing people living off the sea here for thousands of years. Later, fishing families lived permanently on boats and then on floating houses in sheltered lagoons such as Cửa Vạn and Vung Viêng.

From 2014 the province moved most floating-village families ashore to new housing, citing safety and water quality. A few households remain as fishing and pearl-farming communities, and visitors now see the villages mostly by bamboo boat, rowed by former residents.

Hạ Long city grew around the Hòn Gai coal port, and coal is still mined in Quảng Ninh province. Tourism has overtaken it as the image of the province, with Tuần Châu island turned into a cruise harbour and resort.

The Bạch Đằng River enters the sea at the western end of the bay, and it saw two of Vietnam's most celebrated victories. In 938 Ngô Quyền planted iron-tipped stakes in the riverbed and drew a Southern Han fleet in on the high tide; when the tide fell, the ships impaled themselves. In 1288 Trần Hưng Đạo used the same trick against the Mongol fleet.

Remains of the stake fields have been excavated near Quảng Yên, and some stakes are displayed in museums in Quảng Ninh and Hà Nội. The legend of the dragon protecting the bay grew alongside these stories of defending the coast.

In the 20th century the bay was mined by the United States in 1972 to close Hải Phòng harbour, and mine-clearing followed in 1973. Today the naval history is mostly visible in museums rather than on the water.

Overnight cruising in wooden junks began in the 1990s and grew into a fleet of several hundred boats. Routes are licensed by the management board, which is why boats follow the same few itineraries and why moorings, caves and viewpoints fill at the same hours.

Pressure has pushed operators to Lan Hạ Bay and Bái Tử Long, and pushed the authorities to rules on waste water, single-use plastic and boat safety. Standards vary; asking an operator how it handles waste is fair.

Storms are the other limit. Departures are suspended when the port authority orders it, sometimes for days in the typhoon season, as with Typhoon Yagi in September 2024. The bay is at its most reliable from October to June.
