How to apply, step by step
Apply online from outside the UK. You can normally apply up to six months before the date you intend to travel, and you must be outside the UK when you apply and when the decision is made. The application is handled by UK Visas and Immigration; there is no interview in most cases.
You will need a current passport, proof of your savings if asked, and a tuberculosis test certificate if you are applying from a country where the UK requires one. You will also pay the immigration health surcharge, which gives you access to NHS care on the same basis as a UK resident for the length of the visa.
Once granted, you get a visa vignette or an online immigration status. You must enter the UK within the validity window on the visa. The two years run from your date of entry, so do not activate it earlier than you need to.

Two costs dominate: the visa application fee and the immigration health surcharge. The fee is in the region of £298 and the health surcharge is charged per year of the visa, so the total is well over £1,000 (roughly USD 1,300–1,800 depending on the rate). On top of that, budget for biometrics if your application centre charges a service fee, a tuberculosis test if required, and your flight. Inside the UK, the big ongoing costs are rent, transport and the council tax or bills that come with a room in a shared house.

Exceptions and limits
The visa is generous on work but strict on a few things. You can be employed, self-employed, work for multiple employers, change jobs and study. You cannot work as a professional sportsperson or sports coach, and you cannot work as a doctor or dentist in training. Those restrictions are about protecting domestic training routes, not about your qualifications.
You cannot extend the visa, and you cannot switch into another visa route from inside the UK on the basis of this one. If you want to stay longer, you generally need to leave and apply from outside under a different route, such as a skilled worker visa with a sponsoring employer.
Dependants are not allowed. A partner or child cannot come with you on your visa; they would need their own route.
Some nationalities face an annual cap through the ballot. If you are not selected, you cannot apply that year, even if you meet every other requirement.

After you land
You need a National Insurance number to work and pay tax. Apply once you are in the UK; it takes a few weeks, and you can start work while you wait if your employer is willing. You also need a UK bank account for wages, which usually requires proof of address.
Register with a GP surgery as soon as you have an address. The health surcharge gives you access to NHS care, but registration is not automatic and popular surgeries close their lists.
