The Rio San Juan Region is Nicaragua's water region: a river running from Lake Cocibolca to the Caribbean, a Spanish fortress at El Castillo, the Indio Maíz reserve and the Solentiname islands. There are almost no roads — everything moves by boat from San Carlos. It is worth it if you want wildlife, river travel and a slower pace; it is not worth it as a rushed add-on, because the bus from Managua alone is about 6 hours and downriver boats are slow and infrequent.

- The Managua–San Carlos bus is about 6 hours; do not plan to arrive and catch a same-day downriver boat.
- Domestic flights to San Carlos have been unreliable — some years scheduled services did not operate. Treat any flight as unconfirmed until booked directly with the airline.
- Bull sharks live in the river and lake; attacks are rare but the risk exists. Ask locally before swimming.
- Guides are recommended for hiking in remote or reserve areas; getting lost is the main hazard.
