Paraguay's railways were built for freight and have largely been out of passenger service for decades. A short stretch near Asunción has been used for tourist and heritage runs at times, but these are irregular, seasonal or suspended depending on the year, and none of them form a network you can plan a trip around. Treat any train you read about as a possible day out from the capital, not as transport between cities.
That leaves the bus as the practical answer. From Asunción's terminal, Encarnación is 5–6 hours and Ciudad del Este 5–6 hours, both US$10–25 on the better companies (NSA, Crucero del Norte, La Encarnacena), with reclining seats and air-conditioning. Filadelfia in the Chaco is 7–8 hours. Cheaper buses stop everywhere and take longer.
If a heritage train is running while you are in Asunción, it is usually a short return trip of an hour or two, booked locally or through the operator, with a small fare. Because schedules change and services pause, confirm the current status in Asunción before you build a day around it — a hotel desk or the terminal information window is the fastest check.

Tips
Buy intercity bus tickets at the Asunción terminal the day before; for weekends and public holidays, buy earlier. Ask for a coche cama (reclining) seat on overnight or long runs.
