Step by step: how to build the trip
Step 1 — Decide whether Paraguay is the main trip or a stop on an Iguazú route. Short trips work best as part of an Argentina or Brazil route; if Paraguay is the point of the trip, give it 10 days or more.
Step 2 — Lock the spine: Asunción, then Encarnación for the missions. These two carry most of the country's visitor infrastructure and are the safest bet for a first visit.
Step 3 — Add the east only with a purpose. Ciudad del Este is a border trade city; its value is Itaipú, Saltos del Monday and the crossing to Foz do Iguaçu or Puerto Iguazú. If you are not crossing, it is skippable.
Step 4 — Add the Chaco if you have 10 days and travel in May–September. Filadelfia is the base; Loma Plata, Neuland and Mariscal Estigarribia sit further out and need a car or a guide.
Step 5 — Fill the gaps around Asunción with day trips: Areguá for ceramics and the lake, San Bernardino as the summer lake resort.
Step 6 — Book long-distance buses a day or two ahead in high season and keep one buffer day before an international flight.

Notes
Distances are long and roads outside the main corridors are slow; do not stack three regions into a week. The Chaco in particular is a guided, vehicle-based trip, not a place to arrive and improvise.
Heat is the other constraint. Asunción and the south are hot and humid from December to February; the missions are best walked early in the day. The Chaco is only comfortable in the cooler months.
One ticket covers several Jesuit mission sites near Encarnación, so plan Trinidad and Jesús in a single half-day and start early. In Asunción, walk the historic centre in the morning; it empties after office hours.
If you cross at Ciudad del Este, stamp in and out at every bridge. Brazil requires an e-visa for US, Canadian and Australian citizens. Carry travel insurance that covers the countries on your route, not just Paraguay.
