1 week: capital plus one lodge
Seven days buys you one region, not two. Fly into Port Moresby, take a driver for the National Museum, Parliament and Bomana War Cemetery, sleep one night, then fly out to Tufi for fjords and reefs or Goroka for highland culture and coffee. Five nights in one place is enough to settle, dive or walk, and meet people rather than just photograph them.
Return to Port Moresby the day before your international flight. Domestic delays are common enough that a same-day connection is a gamble. If the September Goroka Show is your reason for coming, book accommodation and flights many months ahead; show-week beds in Goroka sell out.

Two weeks lets you add a second region, and the classic pairing is Highlands plus coast. Port Moresby one night, then Goroka or Mount Hagen for three nights, then Kimbe Bay or Tufi for five, then back through Port Moresby. Routings often return via the capital, so treat Port Moresby as a hub you pass through rather than a place you skip.
The alternative 14 days is the Kokoda Track: 8–10 days on foot over the Owen Stanleys, April to November, plus three rest nights in Port Moresby. That is a trek, not a sightseeing itinerary, and it needs its own training and operator booking.
If you want the Sepik instead, run Port Moresby, Mount Hagen or Goroka, five nights on the river (Karawari or by boat), then Madang or Tufi for four. This is expedition pace: Sepik legs rely on charter flights and boats, and weather can move the schedule.
3 weeks: Highlands, Sepik and the islands
Three weeks is where Papua New Guinea stops feeling rushed. One shape that works: Port Moresby 1 night, Highlands 4 nights, Sepik River 5 nights, then the New Guinea Islands — Rabaul and Kokopo for the volcanoes, war tunnels and Mask Festival, or Kavieng for surf and dives — for 8 nights, then Port Moresby 1 night. That is three domestic flight legs, so build in slack.
Time the Highlands with the August Mount Hagen Show or the September Goroka Show, and the islands with the June–October window around Rabaul or the September–December dive season. Show dates and festival calendars move; confirm before you lock flights.
Keep one spare day somewhere in the middle. It absorbs a cancelled domestic flight without costing you the international one at the end.
Notes that change the plan
Domestic aviation is the spine of any Papua New Guinea itinerary. Air Niugini and PNG Air link the main towns, but schedules are thin and change; confirm routes and baggage limits before booking lodges around them. Charter flights are normal for Sepik and some island legs.
Road links between regions are not a realistic plan for most visitors. Where roads exist, they are slow, and security varies by province. Use an operator for transfers in Port Moresby, Lae and Mount Hagen.
